Complete this exercise after completing Lesson 11 - 14 in your Access book.

Create a data entry form for recording the responses to Sweet Lil's Customer Survey. Sweet Lil's is expecting a high response rate, so you want to make the data entry as easy and accurate as possible. Toward this end, when creating your form, you should use combo boxes, a check box, validation checks and default values where appropriate.

The form you create should contain a subform. The main form allows the data entry person to find the customer and then the survey results for that customer can be entered in the subform.

Note: You should however use a combo box for entering the response to Question 2.

When you have completed the form, make sure you save it. You will need to demo its use to the instructor the evening of July 3. Test the form by entering yourself as a respondent. Then generate the following 2 printouts:

1. With your personal data displayed on the new form, print the form (select current page when printing so only one form prints).
2. Using the Documentor (Tools -> Analyze -> Documentor)
   • Print the data properties for the for both your mainform and subform. Include for Form, only properties, and for Sections and Controls, inlcude name and properties
   • Include for Form, only Properties and for Sections and Controls, include Name and Properties. The result should only be 2 or 3 pages at the most. If it is longer, do NOT PRINT. Repeat the steps above to be sure only DATA PROPERTIES was selected.
