Giant Eagle Fundraiser Order Form

Use this form to order your Giant Eagle gift cards that can be used at Giant Eagle and GetGo locations. Order by the last Sunday of the Month. Card can be picked up on the first Sunday of each month or during office hours (M-F, 10 - 2). Others may pick up your cards for you only if you authorize them to do so below. Payment must be made by cash or check at the time you pick up your order.

Monthly Standing Order Commitment
By checking the box above, you are indicating that the order listed above should be replicated eavery month, until you give notice that it should end or change. You understand that you will not need to submit a new order each month, but that you must send/bring payment each month, unless an automatic bank transfer (bill payment) has been arranged.
