Program Forms

Follow the steps below to register for this year's program.

1. Print, complete and return the Program Release Form for Carnegie Mellon. This form is required if your guest is attending a track or activity. The form MUST be received by April 20, 2012.

   Return your signed forms to:
   Dolores Heagy
   Cyert Hall 175

2. Choose a session and track that interests you.
   
   
   • Mind/Body Sessions (for children aged 8-15)
   • College Station Tracks (for children aged 8-15)
     
     
3. Register for your favorite session and track.
   
   
   • Mind/Body Sessions
   • College Station Tracks
     
   
   
4. Print, complete and return the School Information Form to your teacher.
   
   
5. Print and review the Checklist to help you prepare for Take Our Daughters and Sons To Work® day.
   
   
6. Volunteer to help with Take Your Daughters and Sons to Work. Contact Gloria Gruber.
   
   
   • Help with Lunch from noon-1:00pm.
   • Become a Guide from 1:00-4:00pm.
