Complete a separate peer evaluation form for each member of your team, including yourself. As a self-assessment, keep your own evaluation of yourself. Turn in all others as instructed. A copy of this rating form will be returned to the rated team member as feedback.

The purpose of this form is two-fold:

(1) to give constructive feedback to your team members to improve the overall functioning of the team, so your comments and specific examples are strongly encouraged and

(2) to gather information to make any grade adjustments needed based on exceptional or insufficient contributions by individuals on the team, so you should be sure that your feedback is consistent with your overall assessment of the team member’s contributions
